Non-Refundable Registration Fees

 (Registration fees include league fees, insurance, officials, etc. - Rental items are to be returned at the end of season)

(A 10% discount is given for each additional child in the same household)

 

Football Participants - $125.00

(Age as of September 30, 2008 will determine division placement)

Fees include:

Rental of game jersey, game pants,

7-piece pad set, & shoulder pads

[except Flag]);

All participants receive game socks

and a mouthpiece

Flag participants will receive skull caps

Helmets are not included and must be purchased separately

 

Returning Cheerleaders (rostered in 2007) - $165.00

First-time Cheerleaders - $180.00

(Age as of July 31, 2008 will determine division placement)

 Fees include:

Rental of cheering uniform,

tennis shoes, socks, lollies, polo,

sweatshirt, t-shirt and cheering bag

 with participant’s name

Completed applications must have the following:

·  Page 3 [Participant Eligibility and Code of Ethics Form, Zero Tolerance Policy & Addendum #9 (if applicable)] of Registration Record must be completed and signed by football participant and Parent/Guardian*

·  Current picture of registrant must be attached to form

·  First-time registrants (not rostered in 2007) must provide original birth certificate**

·  Page 3 [Participant Eligibility and Code of Ethics Form & Zero Tolerance Policy] of Registration Record must be completed and signed by cheering participant and Parent/Guardian*

·  Current picture of registrant must be attached to form

·  First-time registrants (not rostered in 2007) must provide a copy of birth certificate

**3-page Registration Record is available on GLMYFA website or at on-site registrations

**Original birth certificates are held with Registration Record until Roster Night is completed by MYFL in September
